



Spend the day at our day center and return to your own home in the evening. The Day Center is the bridge to services that help frail elderly, the disabled and people with Alzheimer's disease or those with cognitive impairments, to continue to live at home. Moreover, our day center provides something even the best caregiver can't provide at home: a change of scenery, enabling the participant to get out during the day, socialize, and enjoy stimulating activities. A day at the center helps adults remain in the community, postponing or eliminating admission to a nursing home.
We serve adults with physical disabilities, elderly people who
are frail and can't be left alone and those people who have
been diagnosed with Alzheimer's disease or cognitive
impairment. Some of our participants are supported
with a combination of homecare and day center care
which provides optimal assistance for people who
cannot sustain independent living.
Our participants reflect the diversity of New York City.
Although
most are elderly, we also serve younger individuals who have
been debilitated from injuries or illness. Our Spanish-English
participants feel right at home with our bi-lingual staff.
Most are Medicaid recipients.
The Jacquelyn Hernandez Adult Day Center is a
medical
model, multi-disciplinary program designed to maintain
maximum levels of health
and psychological well-being.
Our brand new, state of the art facility, provides
participants with ready access to: medical professionals,
social workers, nutritionists and recreation therapists.
The high quality services that we have been providing
and are known for, are now delivered in our spacious
and modern new building.
Advanced age, physical disability or Alzheimer's disease can require 24/7 care. It's not always easy to provide that care and still retain a sense of self. At Jacquelyn Hernandez Adult Day Health Center our bi-lingual medical model day program provides respite to caregivers, by caring for loved ones in a stimulating, safe and secure place, thereby providing you, the caregiver, time for yourself.
