When your loved one can't be left at home alone because of frailty or disability, we can help. The Jacquelyn Hernandez Adult Day Health Center is a medical model program that cares for the elderly and disabled during the day so that you, the caregiver, can go to work, take care of errands, or just have some much needed time for yourself. You will have the peace of mind that comes from knowing that your loved one is safe, secure, engaged and under the supervision of a registered nurse and a professional team of caregivers.
Roundtrip transportation is provided so that when you return home, so too does your family member.
At Jacquelyn Hernandez Adult Day Health Center, we restore and maintain the physical and mental health of your relative while improving your family's overall quality of life. A full program of social activities, hot meals, medication management and a wide range of therapies are available.
For the past 14 years, when looking for a community-based program, caregivers have turned to the Jacquelyn Hernandez Adult Day Health Center.
Our Staff is Bi-lingual (Spanish/English)
