Our Place

Jacquelyn Hernandez Adult Day Health Center is an affiliate
of Cobble Hill Health Center. We have been serving the
elderly, disabled and people with Alzheimer’s
disease for over ten years. Our brand new,
spacious, state of the art building, located
in Bushwick, Brooklyn, has been
specially designed to accommodate
the needs of our participants. The
center is a bright and beautiful
environment with special
areas for activities, therapy,
meal service and relaxation.
We can accommodate up to
100 day care participants.

The program runs 6 days
a week (Monday-Saturday)
with a varied and
comprehensive range
of activities suitable for the
elderly, disabled and people with Alzheimer’s disease. In addition, registered nurses are on staff for medical monitoring and medication.

The program, including transportation, is completely covered by Medicaid.

Our One-Stop-Shopping Model of Care

For those who choose to receive all or some of their medical care on site, our one stop shopping model of care provides access to a Board Certified Geriatric physician, Geriatric Psychiatrist, social workers, and on-site pharmacy service eliminating travel to and from appointments.

All of our medical and professional staff are bi-lingual English/Spanish.

 

Our New center is also the home of Cobble Hill Health Center’s Long Term Home Health Care Program. Called “Nursing home without walls", this program provides care to patients with chronic medical conditions at home.
